Name On Backpack: If you would like to have a custom name on the back of the jersey, input in the field box above, "Name on Backpack". Leave field blank if you would prefer no name.
Jersey Number: Jersey number must be a max of 2 characters (Ex. 00, 1, 01, 99, etc.) Leave field blank if you would prefer no number.

Custom team orders will take about 2-3 weeks to receive after cutoff date.  Local (Oahu Residents) Pickup will be coordinated with your team coach.
